It’s pretty hard for people to see things inside of their own mouths, even with lights and mirrors. Now we have small cameras that can be used inside of your mouth to see areas that are even hard for us to see! The video pictures that they take are enlarged and are viewed on a computer monitor right in the treatment room so we can see even the smallest and most well-hidden areas of your mouth.

Intraoral cameras can reveal early stages of potentially serious problems, helping you to make an informed decision about your treatment options. They can be especially helpful for seeing small cracks in teeth, gaps forming around old fillings, for checking on gum disease and for doing an oral cancer examination. The images can be printed or emailed to specialists or insurance companies as needed. Since the images are stored electronically, they don’t degrade over time.

ORAL BACTERIA AND RHEUMATOID ARTHRITIS

Does gum disease indicate future joint problems? Although researchers and clinicians have long known about an association between two prevalent chronic inflammatory diseases—periodontal disease and rheumatoid arthritis (RA)—the microbiological mechanisms have remained unclear.

New research has uncovered how the bacterium responsible for periodontal disease, P. gingivalis, worsens RA by leading to earlier onset, faster progression and greater severity of the disease, including increased bone and cartilage destruction.

Studies indicate that compared to the general population, people with periodontal disease have an increased prevalence of RA and periodontal disease is at least two times more prevalent in RA patients. Other research has shown that a P. gingivalis infection in the mouth will precede RA, and the bacterium is the likely culprit for onset and continuation of the autoimmune inflammatory responses that occur in the disease.

Prevention of periodontal disease can, therefore, help prevent the onset of Rheumatoid Arthritis.

GUM DISEASE AND YOUR HEART

Q: Can gum disease cause other health problems?

A: It’s long been known that your oral health is closely related to your overall health. New research has found that chronic periodontitis, an inflammatory gum disease, is potentially related to the severity of heart attacks, also called myocardial infarctions. Researchers from the University of Granada in Spain studied 112 heart attack patients and tested them for a variety of conditions, including periodontal health. Their preliminary findings were that there is a relationship between gum disease and the severity of heart attacks. Their findings were published in the prestigious Journal of Dental Research under the title, “Acute myocardial infarct size is related to periodontitis extent and severity.” Research is continuing.

Medical experts have known for some time, however, that bacteria from gum infection—and from other ailments in the mouth—can get into the bloodstream and cause problems. Researchers have found, for instance, that people who suffer from gum disease are twice as likely to suffer from coronary artery disease as those without gum disease. Several years ago, as published in the journal Obstetrics and Gynecology, doctors determined that in the case of a 35-year-old mother who had a stillborn birth, the baby’s stomach and lungs contained the same strain of oral bacteria the mother had in her untreated gum disease.

HOW DOES A TOOTH DECAY?

Q: What gets a cavity started?
A: What starts a cavity is the repetition of a process that occurs in your mouth when sugars from the food and drink you consume interact with bacteria that live in the plaque on your teeth. That interaction produces acid, which begins a process called demineralization, the loss of calcium and phosphate from the tooth’s enamel. It usually starts in a spot not readily noticeable, like in a fissure. Eventually, over months and months, the affected patch of enamel collapses, leaving a hole, or cavity. Left untreated, the cavity will spread into the dentin, the softer, more sensitive part of the tooth under the enamel. If the excruciating pain hasn’t driven you to the dentist first, decay will eventually destroy the tooth.
One of the keys to preventing decay is the removal of plaque. Plaque is the filmy substance that’s on the teeth. The more it is allowed to build up, the greater the danger of decay. Brushing and flossing is so important because they are the most effective ways of eliminating plaque. Having your teeth professionally cleaned and examined is essential. No matter how well you brush and floss, some plaque hardens into calculus or tartar and can only be removed by a dentist or dental hygienist. A professional dental examination is your best chance to find cavities and other dental problems before they become more severe too.

• Tooth Whitening brightens teeth that are discolored or stained. There are a number of methods used for bleaching. We use trays and professional-strength bleaching gel for effectiveness both for current usage and for touch-ups in the future.
• Bonding can improve the appearance of chipped, cracked or stained teeth and can close spaces between teeth. Tooth-colored materials are applied or bonded to the tooth surface.
• Veneers are thin custom-made shells designed to cover the front surfaces of teeth. As long as the back surfaces are not cracked, decayed or have worn-out fillings, veneers are a good option.
• Crowns are tooth-colored restorations that restore the front, side and back surfaces of teeth. They can also provide protection for teeth that have become weakened due to large fillings, fractures, and decay.

DENTAL CARE DURING PREGNANCY

The best thing a pregnant woman can do for both herself and her baby is to continue regular dental visits. For one thing, a bacterial infection in the gum could affect the placenta, the organ that links the fetus with the mother’s uterus. There has been some concern over the years about a link between periodontal infection—gum disease—and pre-term, low-birth-weight babies.

Pregnancy is a joyous time. Let your dentist know about your pregnancy at the first opportunity. A woman’s body functions change significantly during pregnancy. Fluctuation in estrogen and progesterone levels and other hormonal changes can affect the body’s response to the bacteria, mentioned above, that cause gum infection. So the need for dental care during pregnancy actually increases. Knowledge of your condition will also affect the decisions the dentist makes regarding the use of certain sedatives, like nitrous oxide, and prescriptions he or she may recommend.

At some point during a nine-month period, you will be due for one of your six-month visits to the dentist. And as your obstetrician or general physician would tell any mother-to-be, the best way to keep an in-utero baby healthy is to keep mom healthy.

TOOTH GRINDING

Q: My husband grinds his teeth at night. Sometimes it drives me out of the room. What can we do?

A: The good news is that, yes, there are steps that can help your husband with his—and your—problem. The technical name for your husband’s condition is bruxism. Some experts estimate that as many as 10 percent of the population deal with this grinding. And that includes children right through the elderly.

The habit is not necessarily confined to nighttime. People who grind their teeth in their sleep generally have no idea they’re doing it. And they may wake up feeling fine, especially if they grind earlier on in their sleep cycle. Others, however, wake with some combination of jaw, shoulder and neck pain. Bruxism also can significantly wear teeth and loosen them. It can crack tooth enamel and chip or break teeth. During sleep, bruxism can cause a person’s jaw to clench at a pressure up to six times greater than the pressure during waking hours.

Causes of bruxism can vary. Stress, a sleep disorder, an abnormal bite or crooked or missing teeth all can be factors. The best person to talk to if you’re suffering from bruxism is your dentist. Whatever the cause, your dentist will know what step to take to deal with it. If it’s stress, for instance, it’s possible that physical therapy or counseling could help. Muscle-relaxing medication or a nighttime mouth guard, specially fitted by the dentist, may be the answer.

PREVENTING TOOTH DECAY

Tooth decay is the destruction of tooth structure and can affect both the enamel (the outer coating of the tooth) and the dentin layer of the tooth.

Tooth decay occurs when foods containing carbohydrates (sugars and starches), such as breads, cereals, milk, soda, fruits, cakes, or candy are left on the teeth. Bacteria that live in the mouth digest these foods, turning them into acids. The bacteria, acid, food debris, and saliva combine to form plaque, which clings to the teeth. The acids in plaque dissolve the enamel surface of the teeth, creating holes in the teeth called cavities.

To prevent tooth decay:

Brush your teeth at least twice a day with a fluoride-containing toothpaste. Preferably, brush after each meal and especially before going to bed.
Clean between your teeth daily with dental floss or interdental cleaners.
Eat nutritious and balanced meals and limit snacks. Avoid candy, pretzels and chips, which can remain on the tooth surface. If sticky foods are eaten, brush your teeth soon afterwards.
Ask your dentist about dental sealants (a plastic protective coating) applied to the chewing surfaces of your back teeth (molars) to protect them from decay.
Drink fluoridated water. At least a pint of fluoridated water each day is needed to protect children from tooth decay.
Visit your dentist regularly for professional cleanings and oral exam.
